Please bring your extended benefits card (with policy number & member ID) and a Doctor’s referral, if required by your policy.
Since all extended health plans are not the same, it is the responsibility of the patient to know the availability of services. To have a better understanding please contact your insurance provider.
Please note: Semiahmoo is only able to invoice one health care provider per visit and is unable to coordinate with numerous payors. Additionally, if the patient has an open insurance claim through ICBC or Worksafe, we are unable to provide direct billing. For any partial coverage or denied claims, it is the responsibility of the patient to pay in full for the services rendered.
